Documents necessary to initiate proceedings for the conferral of a doctoral degree:
1. Request for continuation of proceedings for the award of the doctoral degree download file
2. Doctoral dissertation with appendices (electronic version in PDF file) download file
3. An abstract of the dissertation in Polish (1-page A4) at the top of the page: name, surname, the title of work; at the bottom of the page: signature
4. Dissertation abstract in English (1-page A4) at the top of the page: name, surname, the title of work; at the bottom of page: signature
5. Opinion of the promoter(s) download file
6. Report of the anti-plagiarism system - short protocol
7. Declaration of the doctoral student according to Appendix 1 of the Rector's Decree 90/21 download file
8. An opinion of the Director of the Doctoral School on the doctoral student, certifying the attainment of learning outcomes at PRK level 8, including knowledge of a modern foreign language
9. Copy of diploma confirming possession of a Master's degree or equivalent (original for inspection)
10. Personal questionnaire download file
11. A publication or monograph, which is a statutory prerequisite for the initiation of proceedings
12. Minutes of the presentation of the completed dissertation at a meeting of the Institute/Departmental Council, together with a list of attendees
13. A favourable opinion from the Bioethics Committee or the local Animal Research Ethics Committee or an opinion confirmed by the Committee's Chairman that there are no characteristics of a medical experiment
14. In the case of a dissertation based on a series of publications, a statement by the doctoral student, the supervisor, the promoters, the supervisor and assistant supervisor and the co-authors on their contributions to the work
15. Letter from the supervisor(s) indicating the proposal of 2 candidates to the Doctoral Committee
